Thank you for considering Dreamclinic for your Medical Massage. Please follow the following steps to receive Medical Massage at Dreamclinic:

1. Get your specific massage benefit confirmed by Dreamclinic before making an appointment 

When you are planning to come in for medical massage, call us first at 206-267-0863 so we can get the information needed to confirm your medical massage benefits.  We need to confirm your benefits in advance to ensure that your treatment with us will be covered by your insurance company. There are no exceptions to this policy.

2. Have a Prescription 

Obtain a prescription or written referral for massage therapy from your doctor, chiropractor or other medical professional licensed by the State of Washington to diagnose physical ailments.  Your prescription must be on file before treatment begins.

Information required on prescriptions:

• Date Issued
• Diagnosis and ICD-9 code
• Final number of massages prescribed
• Provider name and physical or electronic signature

* Please also check with your prescribing physician that the prescription is for Massage Therapy and not Physical Therapy. 

3. Schedule Your First Appointment 

After benefits have been verified (which can take several business days), we will call you back and inform you of your benefit details. At that time you are free to make your appointments, or call us back when it is a better time for you. It is important to make sure the massage practitioner you are seeing will be covered under your particular insurance plan. Each time you schedule an appointment, please remind the administrator of your insurance carrier.

4. Complete Our Insurance Paperwork 

Please arrive at least 15 minutes early for your visit to complete the needed insurance paperwork. You will need to do so even if you have been to the clinic before as a cash client. Remember to bring a copy of your prescription/referral and your insurance card or information. You can expect a thorough intake session with your massage therapist before the massage begins; together you will formulate a massage treatment plan that is in accordance with what your physician has prescribed.
